Create Case

On the case form, include as much information as possible, including:

• Location on road of pothole.
  
  • Example: left lane, northbound on Klosterman. 
• Cross streets, intersections, or landmarks that can assist responding technicians. 
  • Example: On Waters between N Manhattan and Lois, same side of road as gas station.
• Estimated depth/size of the pothole.
